“Doctor, do you follow the 3/2 spacing rule?”
It sounds technical, but it’s actually very simple. It’s the biological law that dictates exactly where an implant must be placed to survive. In this guide, we explain this “safety code” and how to check whether your surgeon is following it.
What Is the “3/2 Rule”?
The “3/2 rule” (often derived from Tarnow’s law) refers to the minimum safe distance required between implants and teeth to keep the surrounding bone alive. If an implant is placed too close to another root, the blood supply is cut off — the bone dies, the gum recedes, and the implant fails.
1. The “3 mm” Rule (Between Implants)
- The law: when placing two implants next to each other (as in an All-on-4 bridge), there must be at least 3 mm of bone between them.
- The risk: if a dentist tries to “cram” implants together with only 1–2 mm of space, the bone between them melts away (resorbs).
- The symptom: this creates a “black triangle” — an ugly dark gap between your teeth where the pink gum used to be.
2. The “2 mm” Rule (Between Implant & Tooth)
- The law: there must be at least 1.5–2 mm of space between an implant and your natural tooth.
- The risk: if the implant touches your natural tooth root, it can damage the nerve and kill the healthy tooth.

Why Do Some “Cheap” Clinics Break This Rule?
You might wonder, “why would a dentist ignore this?” Usually it comes down to a lack of planning or outdated technology:
- Freehand surgery: if a dentist places implants “by eye” without a 3D guide, it’s almost impossible to guarantee that perfect 3 mm gap. They might aim for 3 mm but actually drill at 1.5 mm.
- Using the wrong size: cheaper clinics often stock only “standard” size implants. If your bone is narrow, they may force a wide implant in, violating the spacing rule.

The “2-2-2 Rule”: The Healing Timeline
While the 3/2 rule is about space, the 2-2-2 rule is about time. A safe surgical timeline usually follows this rhythm:
- 2 jaws: we treat the upper and lower jaws as one complete system, to make sure your bite is balanced.
- 2 stages: stage 1 is surgery; stage 2 is fitting the permanent teeth.
- 2 months (minimum): we prefer to let the bone heal for at least 2 months before attaching the heavier permanent porcelain teeth.
Warning: be very careful of clinics promising “permanent teeth in 3 days.” While temporary teeth are fine, loading heavy permanent teeth onto an implant before the 2-month mark can break the 2-2-2 rule and cause failure.
How to Check Your Oral Surgeon
You don’t need to be a dentist to check this. When you’re consulting a clinic (whether in Turkey, Spain or the UK), ask for your treatment plan simulation. Top-tier clinics use 3D tomography (CT scans) to plan the surgery virtually before they ever touch you.
- Ask to see the 3D digital plan.
- Ask: “Does this plan guarantee 3 mm of spacing between the implants?”
- Ask: “Do you use a surgical guide?” (a 3D-printed template that physically stops the drill from moving off-target).
