Case Study: Correcting Severe Overjet & Terminal Dentition (Dual All-on-6)

Case Study: Correcting Severe Overjet & Terminal Dentition (Dual All-on-6)

Sometimes, a smile makeover is about more than just white teeth—it’s about fixing the entire architecture of the jaw.

Sean E. came to Smile Dental Turkey with “terminal dentition” (failing teeth) caused by advanced periodontal disease. However, his biggest challenge was functional: the bone loss had caused his upper teeth to flare outward, creating a severe Overjet (Class II Malocclusion) that made eating difficult and ruined his profile.

This is the story of how we used a Dual All-on-6 Implant System to not only replace his teeth but to completely correct his bite.

  • Patient: Sean E.
  • Treatments: Full Mouth Extractions, Dual All-on-6 (12 Neodent Implants), Bite Correction
  • Lead Surgeon: Özkan Özkaynak, D.D.S., Ph.D.
  • Restorative Dentist: Uğur Gültekinler, D.D.S.
  • Restoration: 24 Metal Porcelain Crowns
  • Aesthetic Choice: Colour 0M1 (Bleach White) / Natural Style
  • Total Investment: £8,756.55

The Initial Consultation: The “Drifting” Bite

When Sean first visited us in June 2025, his condition was severe.

  1. Pathologic Migration (Flaring Teeth): As seen in his side-profile photo, his upper front teeth were pushed dramatically forward. This is a common side effect of gum disease—as the bone weakens, the teeth “drift” outward.
  2. Collapsed Bite: He had lost posterior support (back teeth), forcing him to chew on his fragile front teeth, worsening the flare.
  3. Active Infection: Generalised chronic periodontitis meant none of the teeth could be saved.

The Challenge: Notice how far the upper teeth protrude past the lower lip. This “Overjet” makes it impossible to close the lips comfortably or bite into food properly.

Initial OPG: The scan confirms severe bone loss around the roots, explaining why the teeth were loose and drifting.

The Solution: “Resetting” the Jaw

Because the natural teeth were causing the bite issue, extracting them gave our surgeons a clean slate to “reset” the jaw relationship.

Step 1: Clearing the Foundation (June 2025)

We performed full-mouth extractions (21 teeth) to remove the infection. This stopped the “drifting” process immediately.

Step 2: The Surgical Correction (August 2025)

Two months later, once the gums had healed, we placed the dental implants.

  • Dual All-on-6: We placed 6 Neodent (Straumann Group) implants in the Upper Jaw and 6 in the Lower Jaw.
  • Surgical Bite Correction: Unlike natural teeth, implants are placed at precise angles. Our surgeons positioned the upper implants further back and the lower implants strategically to reduce the gap between the jaws.

The New Foundation: 12 implants placed in perfect alignment. Notice how the “flare” is gone—the implants are upright and stable.

Step 3: The Reconstruction (December 2025)

After 4 months of integration, we fitted the permanent teeth.

  • Metal-Porcelain Bridges: Sean chose Metal-Porcelain for its incredible strength.
  • 0M1 Aesthetics: He opted for the brightest “Bleach White” shade (0M1) to complete the transformation from yellow/stained teeth to a dazzling smile.

Final Result: The metal-porcelain bridges are locked onto the 12 implants. The “Overjet” is completely corrected, with the upper and lower teeth meeting correctly.

The Result: A Functional, Aligned Smile

The transformation is life-changing. In the “After” photos, the severe protrusion is completely gone. Sean now has a vertical, healthy bite profile. He can close his mouth comfortably, chew steak, and smile without embarrassment.

Visual Proof: The “buck tooth” appearance is gone. The new smile is straight, bright (0M1), and perfectly aligned with his lips.

Clinical Note: Why Bite Correction Matters

Many patients think protruding teeth just look bad, but they are actually a functional disaster. In Sean’s case, we didn’t just give him new teeth; we surgically corrected his Class II malocclusion. By using a Dual All-on-6 dental Implant system, we had enough support to bring his bite back into a perfect, neutral position. This creates a profile that makes the patient look 10 years younger.

Protruding or Loose Teeth? Start From Home

Sean came to us with failing, flaring teeth and a severe overjet that made eating difficult, and left with a straight, bright, properly aligned bite — his jaw relationship “reset” with a Dual All-on-6. If you have loose, drifting, or protruding teeth, the first step is simple: send us a few photos of your teeth and our specialists will review your case and send back a personalised treatment plan and itemised quote, with no cost and no obligation. Complex bite-correction cases like this are led by our oral surgeon, Özkan Özkaynak, D.D.S., Ph.D., with the final smile designed by our lead cosmetic dentist, Uğur Gültekinler, D.D.S. — and because implant planning depends on your jawbone, your final plan is always confirmed with a 3D scan before any surgery, which, if you’re in the UK, we can even arrange for you before you travel.

Your trip is taken care of, too: any treatment over £3,250 includes a free 6-night hotel stay through our Turkey Teeth Package. You can see more results on our before & after page, or browse more of our patient case studies.
