The Initial Consultation: The Challenge of Severe Bone Loss
When Frederick M. first visited us in February 2022, he was in a difficult situation. His “Before” photo and initial X-rays showed that his few remaining teeth were failing and that he had significant bone loss, especially in his upper jaw where the maxillary sinuses had expanded.
Initial OPG (X-ray) from Feb 2022: This scan shows the few remaining teeth and the severe lack of bone in the upper jaw, making standard dental implant placement impossible.
To get a perfect view of the bone, Dr. Özkaynak ordered a 3D CBCT scan. This scan confirmed we would need to surgically rebuild the bone to create a stable foundation.

A Phased Surgical & Restorative Plan
For a case this complex, a multi-phase approach is essential for a successful, long-term outcome.
Phase 1 (February 2022): The Surgical Foundation
This first visit was dedicated to expert-level surgery.
- 6 Tooth Extractions: Dr. Özkaynak began by removing the 6 remaining failing teeth.
- Bilateral Sinus Lift & Bone Graft: This is the most critical step. Dr. Özkaynak performed a bilateral sinus lift (lifting the sinus membrane on both sides) and placed 2 bone grafts. This procedure rebuilds the lost bone height, creating a strong new foundation for the upper dental implants.
- Temporary Denture: To ensure Frederick M. could eat and smile during healing, Dr. Uğur Gültekinler fitted him with a temporary denture.
Healing Phase OPG (X-ray) from July 2022: This scan shows the patient is now toothless, and the grafted bone in the upper sinuses is healing and integrating.
Phase 2 (July 2022): Dental Implant Placement
After 5 months of healing, Frederick M. returned. The grafted bone was now strong and integrated.
- Dual All-on-6 Placement: Dr. Özkaynak placed 6 Neodent (Straumann Group) dental implants in the newly rebuilt upper jaw and 6 Neodent dental implants in the strong lower jaw. This “Dual All-on-6” system provides incredible stability for a full-mouth restoration.
Post-Surgical OPG (X-ray) from Aug 2022: All 12 dental implants are now in place, 6 in the upper jaw (anchored in the new bone) and 6 in the lower jaw.
Phase 3 (January 2023): The Final Restoration
After a final 6-month healing period for the dental implants to fuse with the bone (osseointegration), Frederick M. returned for his new smile.
- Final Dental Bridges: Dr. Uğur Gültekinler took over the restorative phase. He took final impressions and designed two 12-unit, full-arch dental bridges (totaling 24 metal-porcelain dental crowns).
- Final Fit: These dental bridges were permanently attached to the 12 dental implants, giving Frederick M. a complete, strong, and beautiful new set of teeth.
Final OPG (X-ray) from Jan 2023: This final scan shows the two full-arch dental bridges securely fixed to the 12 integrated dental implants. A perfect result.
