The Initial Consultation: The Problem
Barry visited us in December 2023 with “terminal dentition.” As seen in his “Before” photo and initial X-ray, almost all of his natural teeth were failing due to severe decay and bone loss. He was unable to chew comfortably and was unhappy with the aesthetics of his smile.
The initial X-ray confirmed that none of the existing teeth could be saved. Furthermore, the 3D CT scan showed that his upper jaw had reduced bone height, meaning standard implants wouldn’t work without assistance.
3D CBCT Planning Scan: This scan allowed Dr. Özkaynak to measure the sinus floor and plan for a Sinus Lift to ensure the upper dental implants would be stable.
A 3-Visit Full Mouth Reconstruction
Visit 1 (December 2023): Clearing the Infection
The first step was to remove the failing teeth and prepare the mouth for healing.
- Extractions: All remaining teeth were extracted (13 Upper, remaining Lower).
- Temporary Dentures: Barry was fitted with immediate temporary dentures to ensure he wasn’t without teeth while his gums healed.
Visit 2 (November 2024): The Surgical Phase
Once the gums had healed, Barry returned for the implant placement.
- Upper Jaw (All-on-6): Dr. Özkaynak performed a Bilateral Sinus Lift and Bone Grafting to create a solid foundation. He then placed 6 Neodent (Straumann Group) implants.
- Lower Jaw (All-on-4): 4 Neodent implants were placed in the lower jaw, where the bone was naturally stronger.
Post-Surgical X-ray: This scan shows the 10 total implants placed in the jawbone, awaiting integration.
Visit 3 (November 2025): The Final Smile Makeover
After the implants had fully integrated (osseointegration), Barry returned for his permanent teeth.
- Metal-Porcelain Bridges: Instead of Zirconium, Barry opted for Metal-Porcelain crowns fused into bridges. This is a highly durable and cost-effective option.
