When a patient presents with severe bite misalignment and significant aesthetic concerns, the treatment plan must be carefully staged — protecting their confidence while rebuilding their oral health from the ground up.

Our patient, G.A.R., came to our clinic with high aesthetic worries and complex functional issues. Her lower teeth had severe biting angles, and her upper jaw required significant structural work. To address her aesthetic concerns immediately while ensuring a permanent, functional result, surgical lead Özkan Özkaynak, D.D.S., Ph.D. and cosmetic lead Uğur Gültekinler, D.D.S. designed a highly customised, 3-visit treatment protocol.

The 3-Phase Treatment Timeline

Because of the patient’s aesthetic worries, we prioritised her smile immediately, using a long temporary bridge to mask the surgical phases while the jawbone healed.

Phase 1: Immediate Aesthetics & Foundation Prep (February 2025)

The first priority was to resolve the failing teeth and give the patient a smile she could feel confident in straight away.

  • Extractions & grafting: Özkaynak extracted six compromised teeth (one surgical, five standard). To prepare the upper jaw for future implants, he performed bone grafting at three sites and two sinus lifts.
  • Immediate smile restoration: Gültekinler designed a long bridge using 7 metal-porcelain crowns, so the patient could leave the clinic with a fully functional smile (matched to a natural 2M3 shade) while the bone grafts healed underneath.

Phase 2: Dental Implant Surgery (August 2025)

Six months later, the patient returned for the surgical phase.

  • Implant placement: Özkaynak placed 6 Neodent (Straumann Group) implants into the newly regenerated upper jawbone.
  • Aesthetic maintenance: to protect the healing implants and maintain her confidence, the team adapted her existing metal-porcelain bridge to sit over the implants throughout osseointegration.

Phase 3: Bite Correction & Final Zirconium (February 2026)

During the final visit, the team addressed the lower jaw to make sure the finished bite was perfectly aligned.

  • Structural bite correction: because of the severe biting angles of her lower teeth, Gültekinler performed 10 root canal treatments and placed 6 white fillings. This safely altered the angulation of the lower teeth without any extractions, creating a balanced biting plane.
  • The final restoration: with the foundation perfected, Gültekinler anchored 11 zirconium porcelain crowns to the upper jaw and 7 to the lower jaw.
  • The aesthetics: the final smile was crafted in a brilliantly bright 1M1 shade using a Natural style with softened canines. A custom night guard was also provided to protect her new smile from night-time grinding.
