Case Study: Rebuilding a Worn Smile

Years of severe teeth grinding (bruxism) can cause devastating wear to the natural dentition, essentially grinding the front teeth down to the dentin. Rebuilding a smile with this level of attrition requires both a complete restorative overhaul to restore the bite height and a robust implant foundation to replace unsalvageable teeth.

Our 65-year-old patient, B.S.D.B., traveled to us from the United Kingdom facing exactly this challenge. His top and bottom front teeth were severely worn down by grinding. Additionally, his bone structure on the upper left side lacked the volume necessary for immediate dental implants. To ensure a permanent, highly successful result, we structured his treatment into a careful 3-phase timeline, allowing his bone grafts to heal before placing the final dental implants.

The 3-Phase Treatment Timeline

Because of the patient’s bone structure, placing all the dental implants at once was not clinically viable. We utilized a delayed-loading approach for the upper left quadrant to prioritize safety and long-term stability.

🛑 Phase 1: Initial Extractions, Grafting & Foundation (May 2025)

The first visit focused on removing the most compromised teeth and beginning the foundational rebuild.

• Extractions: One severely worn front tooth in the upper jaw and one in the lower jaw were safely extracted.
• Bone Reconstruction: To correct the bone deficiency in the upper jaw, we performed 2 sinus lifts and applied 3 units of bone grafting material.
• Initial Implants: We strategically placed the first 4 Neodent (Straumann Group) Implants—2 in the upper arch and 2 in the lower arch.
• Immediate Function: The patient was fitted with temporary dentures to maintain his smile and function while the extensive bone grafts healed.

⚙️ Phase 2: Delayed Implant Placement (November 2025)

Six months later, after the sinus lifts and bone grafts had successfully integrated and hardened, the patient returned.

• Secondary Surgery: With the upper jaw now robust enough to support titanium, we placed the remaining 3 Neodent Implants into the newly grafted bone.
• Temporaries: New provisional dentures were provided while these final three dental implants integrated with the jawbone.

🌟 Phase 3: The Restorative Masterpiece (March 2026)

After all 7 dental implants were fully fused to the bone, the patient returned for his permanent smile.

• Restoring Health & Symmetry: We performed gum contouring on the upper jaw to frame the new teeth perfectly, and placed 5 white fillings to restore the remaining natural, salvageable teeth.
• The Final Crowns: We designed and permanently anchored 13 Metal Porcelain Crowns to the upper arch and 14 Metal Porcelain Crowns to the lower arch. Metal porcelain provides the extreme bite strength necessary for a patient with a history of heavy grinding.
• The Aesthetics: To ensure a highly authentic, age-appropriate look, the patient selected a Natural Style shape in the 1M2 shade—a beautifully warm, realistic white.
